Introduction to Microsoft Entra ID Protection

Attackers no longer rely on breaking in through technical exploits. Instead, they focus on logging in with stolen credentials. It’s how modern breaches start. Credentials are for sale. Multifactor authentication (MFA) fatigue is a known exploit path. And once an attacker looks like a legitimate user, many traditional defenses can’t tell the difference.

In fact, according to CrowdStrike’s 2026 Global Threat Report, the average eCrime breakout time dropped to 29 minutes in 2025, a 65% increase in speed from 2024. The fastest eCrime breakout time in 2025 was only 27 seconds. Identity has become one of the fastest-moving threat surfaces in modern enterprise.

Microsoft Entra ID Protection helps organizations detect and respond to identity-based risks. It uses machine learning and behavioral signals to identify risky sign-ins, compromised credentials, and suspicious identity activity. Integration with Microsoft Conditional Access enables risk-based access decisions and automated remediation when identity risk is detected. Rather than waiting for attacks to succeed and then responding, this approach builds defenses around the identity itself.

Let’s break down how it works and why every security team should be treating identity as critical infrastructure.

What is Entra ID Protection?

Entra ID Protection is a security capability within Microsoft Entra ID that detects, investigates, and helps remediate identity-based risks. It continuously evaluates signals associated with users and sign-in patterns to identify suspicious activity and calculate identity risk.

This approach catches what traditional authentication methods miss: legitimate credentials used by unauthorized users. It doesn’t just verify the password; it challenges whether the behavior behind that password makes sense. Signals such as a sign-in from an unfamiliar location, an anonymous or malicious IP address, or unusual sign-in properties can contribute to Entra ID Protection’s assessment of identity risk.

The power of Entra ID Protection multiplies through integration. Microsoft Entra ID is a cloud-based identity and access management service used to authenticate users and manage their access to Microsoft 365 and connected third-party cloud applications. Entra ID Protection extends this role by using identity risk signals with Conditional Access and other Microsoft security tools, allowing identity risk signals to inform access decisions, investigations, and remediation workflows.

Instead of juggling fragmented alerts across disconnected systems, teams get one consolidated picture of where identity risks are and what’s being done about them.

Key capabilities

Entra ID Protection delivers identity security through three core capabilities (risk detection, automated remediation, and risk reporting) that operate together to help organizations identify, investigate, and respond to identity risk. Each plays a distinct role in the identity attack lifecycle, from spotting suspicious behavior to taking action and informing future policy decisions. Here’s how each capability contributes to detecting and stopping identity threats:

Risk detection
Entra ID Protection continuously analyzes sign-in attempts and user activity to detect indicators of compromise (IOC). Entra ID Protection analyzes sign-in and account activity for indicators of potential compromise, including leaked credentials, anonymous or malicious IP addresses, impossible travel, unfamiliar sign-in properties, and other anomalous activity.

Automated remediation
Entra ID Protection risk signals can be used with Conditional Access policies to automatically respond to risk. Depending on the policy and type of risk, organizations can block access, require multifactor authentication, require reauthentication, or require the user to remediate account risk.

Risk reports
Built-in dashboards give security teams visibility into risky users, suspicious sign-ins, and threat trends across the environment. Reports are filterable by risk level, geography, application, and user, which makes it easier to prioritize investigations and tighten policies.

Together, these capabilities form a continuous identity defense loop: detect abnormal behavior, take immediate action, and validate defenses with data.

Implementing Entra ID Protection

Implementing Entra ID Protection successfully requires careful planning, a clear understanding of your organization’s unique risk tolerance, and thoughtful policy design. Push policies too aggressively, and your users will hit constant roadblocks. Set them too loosely, and attackers slip in unnoticed.

The best approach follows a phased rollout. Start by monitoring and alerting to learn how the system interprets your environment. Once you build confidence, gradually enable automated responses to stop threats without disrupting legitimate users. This measured path helps balance security and productivity.

Defining risk policies

Next, define your risk policies. These policies establish how Entra ID Protection responds to different threat scenarios and form the foundation for your identity defense strategy. Balancing security and user experience is critical. Your policies should block or challenge attackers without adding unnecessary steps for legitimate users.

Entra ID Protection provides user and sign-in risk signals that can be used in Conditional Access policies:

  • User risk-based Conditional Access policies
Evaluate the probability that a user’s account is compromised, based on signals such as risky sign-in behavior or leaked credentials, and trigger automated actions like requiring a secure password change or blocking access when the user’s risk level crosses a threshold.
  • Sign-in risk-based Conditional Access policies
Evaluate each login attempt individually to identify immediate risks such as impossible travel or sign-ins from malicious IP addresses. These policies block access or require stronger authentication when necessary.
  • MFA registration policy
Require users to register for multifactor authentication at sign-in, so new users are ready for MFA on their first day. MFA is also one of the ways users can remediate their own risk, which reduces help desk calls.

Conditional Access integration

The real power of Entra ID Protection unfolds through its integration with Microsoft Conditional Access. Microsoft Entra ID Protection works with Microsoft Conditional Access to apply dynamic, context-aware policies that assess access based on multiple risk signals simultaneously. Organizations can connect on-premises identities to Microsoft Entra ID and apply cloud access policies to supported sign-ins, helping extend identity controls across hybrid environments.

Conditional Access can evaluate multiple conditions and signals, including:

  • User risk and sign-in risk scores from Entra ID Protection
  • User workload or identity
  • Device state and compliance
  • Target resources
  • Location and network conditions
  • Authentication context

By layering these signals, Conditional Access makes split-second decisions to grant, challenge, or block access. This process enforces zero trust security principles with precise control over every authentication request.

For example, a trusted user signing in from a registered device at their usual office location may be granted access without additional checks. If that same user attempts to sign in from a location they haven’t accessed from before, such as a different country, or uses a device that fails compliance checks, the system may prompt for multifactor authentication or block access entirely. This approach lets legitimate users sign in without extra steps while raising strong barriers for attackers.

Best practices for secure identity protection

Strengthening authentication

Protect human and workload identities appropriately
Require strong, phishing-resistant authentication for human users, especially administrators and other privileged accounts. For workload identities such as applications and service principals, use workload identity risk detection, Conditional Access where supported, least-privilege permissions, and strong credential and secret management.

Move to passwordless authentication
Passwords remain the weakest link in authentication chains. Replace passwords with phishing-resistant methods like FIDO2 security keys and passkeys, Windows Hello for Business, or certificate-based authentication. These approaches reduce reliance on passwords and strengthen resistance to credential phishing and password-based attacks. Entra ID supports a phased rollout, so you can enable passwordless authentication gradually by group, department, or user role.

Ban known compromised or weak passwords
Use Microsoft’s global banned password list and add custom entries specific to your organization, such as company names or internal project terms. Enforce password protection policies at the domain level to block the use of weak or guessable credentials before attackers can exploit them.

Ongoing risk monitoring

Use the Entra ID Protection dashboard to track identity risk in real time
The dashboard highlights user and sign-in risk levels using behavioral analytics, threat intelligence, and known attack patterns. Focus on users flagged with high or medium risk to take action quickly and limit potential damage. Dig into sign-ins from unfamiliar locations, anonymous IPs, or unusual client apps to understand the threat.

Tune user and sign-in risk policies regularly
Adjust thresholds and actions based on your threat model. For high-sensitivity environments, such as healthcare and finance, consider blocking access at medium sign-in risk, not just high. If certain roles require exceptions, allow overrides but ensure those events are logged and monitored closely.

Regularly audit policies and update them based on threat intelligence
Security threats evolve constantly, so make it a routine to review and adjust your user and sign-in risk policies in response to the latest threat intelligence and attack trends. This proactive and ongoing tuning helps you close gaps before attackers find them and ensures your defenses stay one step ahead.

User training and awareness

Educate users on evolving credential attack techniques
Attackers target your users because they offer the easiest path into your systems. Show them how attackers steal tokens, trick MFA with fake prompts, or bypass protections through OAuth consent phishing and session hijacking. When you give your users real, practical knowledge, they can become part of your frontline defense.

Encourage use of self-service password reset and MFA registration portals
Turn on self-service password reset (SSPR) and tie it tightly to Entra MFA registration. This not only lightens the load on your help desk but also empowers users to take control of their own security. Require users to re-register authentication methods when compromise is suspected or existing authentication methods can no longer be trusted.

Build secure onboarding/offboarding workflows

Complement Entra ID Protection with Microsoft Entra ID Governance capabilities such as Entitlement Management to manage the access lifecycle with clear policies for contractors, interns, and temporary workers. Automate expiration and revocation of access at the end of their engagement to ensure no entry points remain open by mistake.
